5D Cancer Services ("we," "us," or "our") is a covered entity under the Health Insurance Portability and Accountability Act of 1996 (HIPAA) and its implementing regulations. We provide adaptive radiotherapy and related oncology services. This Notice applies to all protected health information (PHI) we create, receive, maintain, or transmit in connection with your care.
PHI is any individually identifiable health information — including demographic data — that relates to your past, present, or future physical or mental health; the provision of health care to you; or payment for your health care. PHI includes information in any form: oral, written, or electronic.
Our hosting infrastructure automatically records standard server log data when you visit this site: truncated IP address (last octet removed), browser type and version, operating system, referring URL, pages visited, and timestamp. This data is used solely to diagnose technical errors and understand aggregate traffic patterns. It is not linked to any personally identifiable information and is retained for a maximum of 90 days before automatic deletion.
This website does not currently use any analytics service. No tracking scripts, third-party analytics tags, or behavioural measurement tools are loaded on any page. We review this position periodically; if analytics are introduced in the future, this policy will be updated before any tool is deployed.
If you contact us by telephone, web form, or email, we may collect your name, phone number, email address, and the content of your message. This information is used solely to respond to your enquiry and is not considered PHI until you formally become a patient and we begin providing clinical services to you.
